An opportunity exists for a qualified and experienced Specialist Support Coordinator located in the South/East region of Melbourne. The role of the Specialist Support Coordinator is to provide Support Coordination to participants with complex needs and assist them in achieving goals as set out in their NDIS plans. The role focuses on addressing barriers and assisting participants.

A part time opportunity exists for a qualified and experienced Specialist Support Coordinator located in the South/East region. The role of the Specialist Support Coordinator is to provide Support Coordination to participants with complex needs and assist them in achieving goals as set out in their NDIS plans. The role focuses on addressing barriers and assisting participants.


You will:

· Provide time limited specialist support coordination to address high level risk and assist the participant to connect with supports and build capacity and resilience

· Actively develop, implement, monitor and review participant's NDIS plans and report on progress and outcomes

· Develop intervention plans for implementation by disability and mental health support workers

· Work collaboratively with external stakeholders and providers to ensure productive community and inter-agency relations

· Use a strength-based and person-centered approach to build skills capacity of participants, ensuring their goals & choices drive the process of engaging supports

· Has either professional or lived experience with people with autism spectrum disorder, cognitive disabilities, acquired brain injuries or complex mental health needs

Essentials

· Current registration with the relevant industry board of Australia

· Tertiary Qualification in an allied health discipline (Social Work, Physiotherapy, Occupational Therapy, Psychology or Speech Pathology)

· Knowledge & experience of NDIS funded plans with minimum 2 years’ experience in a specialist support coordination role or similar

· Pre-existing knowledge, connections & networks of local social & community services available to people with NDIS funding

· Clear NDIS Worker Screening Check, WWCC,  check plus a valid driver's licence & the right to work in Australia
