Are you a passionate Team Leader wanting to join a program that aims to support individuals in the public housing system? We are seeking a team leader to join our Bunbury office!

Contract: Full time

Location: Bunbury Office Contract expires 01/10/2024 (extension subject to program contract renewal)

Thrive team and lead a team to do it! Outcare is delivering the Thrive program services to the Perth metropolitan area, Bunbury and Albany. The program aims to support Aboriginal housing and individuals in the public housing system, particularly those in need of social and economic support.

The services provided take on a holistic approach in identifying client's needs, with a strong focus on early intervention.

Key responsibilities:

  • Actively embraces and integrates our purpose and values into the role, and models appropriate behaviors.
  • Manages a caseload of clients in line with their person-centred plans.
  • Develops person centred plans and accompanying documentation for allocated clients:
  • Assess allocated clients; prepare risk assessments; liaise with client, team members and external service providers; prepare person-centred plans that are co-designed with the client (and their family where appropriate); uses appropriate intervention models and strategies; and reviews progress against plans. Lead and supervise a diverse, engaged and productive team to achieve client and organizational goals.
  • Maintain safe environment for self, team, colleagues and clients by actively assessing and mitigating risks, reporting incidents, and continuously promoting safe work practices including professional boundaries.
  • Build collaborative, constructive relationships with clients, manager, colleagues, and external agencies and others to meet mutual goals and objectives resolving conflict as necessary.
  • Ensures self and team complete administrative tasks including documentation, audits reporting and risk assessments in an accurate and timely manner.

What you'll need:

  • Current WA 'C' Class Driver's
  • License First Aid Certificate
  • Current Working with Children Card
  • Proof of COVID-19 Vaccination in line with government mandate
  • Desirable Experience in working with vulnerable groups with diverse linguistic backgrounds and complex needs, in supporting the clients for a better life.
  • Demonstrated contemporary understanding and attitude of homelessness and tenancy issues Knowledge and experience in delivering services in the context of property management
