Pre-Employment Medical
To be eligible to receive an offer of employment with Diverseco, you must complete a Pre-Employment Declaration. You may also be required to agree to other pre-employment processes. The information derived in this declaration and other pre-employment processes is necessary to assess the suitability of applicants to be offered employment with Diverseco having regard to an applicant's ability to perform the technical aspects of a role and in consideration of the ethical obligations on our employees. Some information is necessary to seek to ensure that we comply with their obligations under the Work Health and Safety Act 2012. Some information is necessary in seeking to ensure Diverseco collects diversity statistics as part of our commitment to collaborating with some of our government agencies for either tenders or government funding. The information is collected and will be managed in accordance with the State Records Act 1997 and destruction schedules issued under that Act. Note that misconduct includes making a false statement in connection with an application for employment. Thus, in the event you are employed, any false statement by you in connection with your application for employment will amount to misconduct and render you liable to disciplinary action, including the possibility of termination of employment. This Pre-Employment Declaration is issued by the National Director Legal and Human Resources in accordance with instruction from the Diverseco Board of Directors.
Pre-Employment Health Declaration
Employment is conditional upon the applicant being fit and fully able to perform the inherent requirements of the role. When completing this declaration, it must be in full knowledge of the position as outlined in the role description. It is a requirement that all applicants applying for roles within the organisation completes a Pre-Employment Health Declaration which is a confidential document. You must complete all components of the Declaration, including providing additional, supporting information and documentation where indicated. Failure to make a disclosure, or the making of a false or misleading disclosure, may disentitle you to compensation pursuant to the Work Health and Safety legislation [pursuant to the state of employment] should you suffer any recurrence, aggravation, acceleration, exacerbation or deterioration of a pre-existing injury or disease arising from employment with our organisation. We may rely upon any failure to disclose in accordance with the provisions of the legislation as grounds for denying compensation. This pre-employment health declaration also assists us in obtaining information to enable us to meet our obligation under the Equal Opportunity Act 2010 to make reasonable adjustments for an employee or prospective employee in order to perform the genuine and reasonable requirements of the employment. Note that misconduct includes making a false statement in connection with an application for employment.
