About the role

Community Connections at Leisure Networks are specialised Support Coordination services that support people with an NDIS plan to connect to their communities and plan for their future. 

As a Support Coordinator, your primary focus is capacity building across all domains of an individual’s life working with customers ranging from children, young people to adults.

Key responsibilities for the Support Coordinator:

  • Empowering customers to exercise their choice and control to access a range of formal and informal mainstream and community-based support services.
  • Build strong and effective relationships with external provider organisations.
  • Monitoring of NDIS plan budget and support services to ensure plan budget is utilised effectively and services are working towards achieving their plan goals.
  • Achieve set targets, KPI's and caseload in a timely and effective manner. 

About you

To be successful in this role you will have:

  • Demonstrated experience in NDIS Support Coordination including sound knowledge and understanding working within the NDIS
  • Experience utilising and knowledge of a range of community-based supports and methods to achieve customer goals and maximize opportunities through their NDIS plan.
  • Demonstrated ability to build rapport and strong working relationships with customers by following through with commitments, being fully engaged, adaptable, efficient and going the extra mile to always provide a professional and ethical service that promotes choice and social and community inclusion
  • Strong written communication skills including being able to write accurate, clear and informative reports and other communication that meets the needs of the intended audience.
  • Proficient use of IT systems and processes including Microsoft, Outlook, internet and email applications.

In addition, you have:

  • Current Victorian Driver License and a well-maintained vehicle that meets current safety standards (VicRoads registered) 
  • Working with Children Check (employee) or willing to obtain
  • NDIS Worker Screening Check (or willing to obtain)
  • Evidence of COVID 19 Vaccinations (3 doses)
