Nextt Group is looking for a Support Coordinator for a full-time permanent position in our Chippendale, Sydney office.

ABOUT THE ROLE

We are seeking to employ an experienced Support Coordinator in our Sydney office on a full-time permanent position to manage a portfolio of customers including all aspects of their service experience and be the key contact point for customer relationship. To work collaboratively with the Regional teams, Customer Service Team and Business development staff to identify customer preferences and match these with appropriate staff & supports. To be responsible for the implementation of participants plans that meet their needs and expectations for private clients and those within allocated NDIS funding.

To coordinate the provision of high quality, individualised and responsive support services to the people that access Nextt’s services. This requires the coordination and supervision of Community Support and/or Care Workers and working collaboratively with the individual, their informal and formal supports, and other stakeholders.

  • Responsible for onboarding new customers to Nextt
  • Responsible for seeking regular customer feedback to ensure quality end to end service experience is achieved
  • Responsible for communication and organizational relationship with customers, families, and external supports 
  • Responsible for providing the requirements brief to Customer service team regarding workforce matching and rostering
  • Responsible for timely implementation of client Plans in line with customer requests and clients with NDIS funding allocation 
  • Responsible for Service Agreement with customer and managing service bookings on Portal
  • Accountable for ensuring that all customer information is captured, updated, and maintained in the various organizational management systems
  • Responsible for implementing and monitoring all internal policies and procedures with regards to customers
  • Develop and review of relevant individual planning documents and tools within the required timeframes

WHAT DO YOU NEED TO HAVE?

  • Demonstrated experience in managing the delivery of customer support plans within a disability framework 
  • Minimum 2 years in a customer interaction role in the disability sector or related 
  • Ideally tertiary qualifications in a related discipline in Health, community, disability, or business
  • Demonstrated experience in customer service and managing customer support plans 
  • Experience in the NDIS environment desirable 
  • Demonstrated experience of the intake / assessment function, planning and service delivery within a disability environment
