About the Role

We are seeking an experienced and passionate Senior Case Manager to join our Thrive Program, supporting individuals to maintain housing stability, build life skills, and strengthen their capacity to live independently.

As the Senior Case Manager, you will provide high-quality, person-centred case management services while offering day-to-day leadership and support to a team of Case Managers. You will play a key role in ensuring safe, effective and consistent service delivery, helping clients achieve positive and sustainable outcomes.

Key Responsibilities

Client Assessment & Case Management

  • Conduct client assessments and identify support needs and risks.
  • Develop and implement person-centred support plans.
  • Provide ongoing case management and goal-focused support.
  • Advocate for clients and connect them with appropriate services. 

Leadership & Service Delivery

  • Provide guidance, supervision and operational support to Case Managers.
  • Support high-quality service delivery aligned with program requirements.
  • Promote trauma-informed, strengths-based and culturally responsive practice.
  • Foster positive client engagement and outcomes. 

Stakeholder Engagement

  • Collaborate with internal teams and external service providers.
  • Participate in case conferences and coordinated service responses.
  • Build effective partnerships to support client outcomes. 

Documentation & Compliance

  • Maintain accurate case notes and client records.
  • Contribute to reporting and monitoring activities.
  • Ensure compliance with organisational, contractual and legislative requirements.

About You

To be successful in this role, you will bring:

  • Experience supporting people with complex needs within community services, housing, justice, or related sectors.
  • Demonstrated experience in case management and person-centred planning.
  • Strong understanding of trauma-informed and strengths-based practice.
  • Ability to manage complex caseloads and coordinate support services.
  • Excellent communication, documentation and stakeholder engagement skills.
  • Experience mentoring, coaching or supporting staff (highly regarded).
  • Strong commitment to client wellbeing, cultural safety and ethical practice.

Essential Requirements

  • Relevant qualification in Social Work, Community Services, Counselling or a related discipline (or equivalent experience).
  • Current Driver's Licence.
  • NDIS Check 
  • NDIS Modules
  • First Aid and CPR
  • Ability to obtain and maintain a Working with Children Check and NDIS Worker Screening Check if required.
  • Australian citizenship, permanent residency, or a valid work visa
